Requesting Medical Records

Requests for access to medical records can be made through a remote website portal for quicker processing and transfer.

Requesting Imaging Studies

Access to view your X-ray, MRI, or CT images electronically can be acquired by request.  Let a clinical team member know you would like to have access to your images and an email will be initiated from which you can follow the below steps.   Images will be available for 60 days.

  1.  You will receive an email from no-reply@medstrat.com stating “OrthoIllinois shared a study with you” (if you do not see the email in your In Box, be sure to check your spam folder).
  2. Click on the blue rectangular button that says “View Study.”
  3. A new tab will open, asking you to enter your Date of Birth.
  4. Check the box to confirm that you have read and agreed to the terms.
  5. Click the blue button that says “Log In.”
  6. The page will refresh and your images will appear.

Surprise Billing Disclosure

Medication Recycling

Help keep water supplies safe by recycling your unused or expired prescription and over the counter drugs.  Never flush, or dispose of medication down a sink or in garbage.  Walgreens currently provides free, secure drug drop off kiosks in three of their area stores year-round. Date and drop off locations for Keep Northern Illinois Beautiful‘s annual drug collection are available on its website.  Other Rockford-area drop off collection sites for expired or unused medications include:

MEDICINE AND SHARPS: The Illinois Drug Take-Back Act went fully into effect on December 1, 2023, requiring pharmaceutical companies to fund programs for medication take-back kiosks in McHenry County for old and unwanted prescriptions. When disposing of sharps, put them in a strong, plastic container, such as a detergent jug or a designated sharps bin. When ready to drop off, seal the container with duct tape.
