Mailed Payment Remittance Notice
Effective October 1, 2025, the mailing address for payment submission for two OrthoIllinois billing accounts will be changing. The new payment remittance addresses are as follows:
- For OrthoIllinois medical services statements please mail payment to OrthoIllinois PO Box 4545 Carol Stream, IL 60197-4545
- For OrthoIllinois Surgery Center - Rockford statements, please mail to OrthoIllinois Surgery Center - Rockford PO Box 4569 Carol Stream, IL 60197-4569
