Patellar and Quad Tendon Tear

Patellar and quadriceps tendon tears are serious knee injuries that affect the tendons responsible for straightening the leg. Tendons are strong bands of tissue that connect muscles to bones.

A quadriceps tendon tear occurs above the kneecap where the tendon attaches the quadriceps muscle to the patella (kneecap). A patellar tendon tear occurs below the kneecap where the tendon connects the patella to the tibia, the lower leg bone. Both injuries disrupt the knee’s ability to extend or straighten properly. In severe cases, the tendon may completely rupture, making it difficult or impossible to lift the leg or bear weight normally.

These injuries happen suddenly during sports, exercise, falls, or other traumatic events. They are more common in active adults, athletes, and individuals over the age of 40, although younger people can also experience tendon tears, especially during explosive athletic activities.

Patellar tendon tears usually occur when a significant force is placed on the tendon. This can happen after a fall, direct blow to the front of the knee, or sudden forceful movement involving the leg.

These tears are commonly associated with jumping and landing activities. The injury may occur when the foot is firmly planted and the knee is bent, such as during a jump, landing, sudden stop, or rapid change in direction. The force generated through the tendon during these movements can cause the tendon fibers to rupture.

Quadriceps tendon tears most commonly occur when the knee is bent and the quadriceps muscle suddenly contracts with significant force, such as during a fall, missed step, sudden change in direction, or attempt to catch oneself from falling. The force placed on the tendon can cause it to partially or completely tear, particularly in tendons that have gradually weakened over time from age-related wear, chronic overuse, or underlying degeneration.

Tendon weakness due to chronic overuse, repetitive stress, or longstanding inflammation can damage tendon fibers and reduce their strength. This process is called tendinopathy or tendon degeneration. Many people experience pain or stiffness in the knee for weeks or months before the tendon finally tears.

Certain medical conditions may increase the risk of tendon tears. Diabetes, kidney disease, rheumatoid arthritis, gout, and chronic inflammatory disorders can affect tendon health. Long-term steroid use, certain antibiotics such as fluoroquinolones, and previous knee surgeries may also weaken tendons. Obesity and smoking may further increase the risk by impairing circulation and tissue healing.

Athletes who perform repetitive jumping, sprinting, or heavy lifting place repeated stress on the knee tendons. Over time, microscopic damage can accumulate and eventually lead to a tear. Poor flexibility, muscle imbalance, and inadequate conditioning may also contribute to injury risk.

The symptoms of a tendon tear can vary depending on whether the injury is partial or complete. In many cases, patients describe a sudden popping or tearing sensation at the time of injury. This is often followed by immediate pain and difficulty using the leg.

Pain is typically located at the front of the knee. With a quadriceps tendon tear, pain is usually felt above the kneecap. With a patellar tendon tear, pain is generally located below the kneecap. Swelling and bruising often develop quickly after the injury.

One of the most important symptoms is weakness when trying to straighten the knee. Patients may notice difficulty walking, climbing stairs, or standing from a seated position. In complete tears, the knee may suddenly give out or collapse because the tendon can no longer support normal movement.

The inability to perform a straight leg raise is a major sign of a complete rupture. Some patients also notice a visible deformity around the kneecap. In quadriceps tendon tears, the kneecap may sit lower than normal because the upper tendon is no longer pulling it upward. In patellar tendon tears, the kneecap may appear higher than normal because the lower tendon is detached.

Partial tears may produce milder symptoms. Patients may still be able to walk but experience pain, weakness, stiffness, or instability. Swelling and tenderness often worsen with activity.

Without proper treatment, complete tendon tears can lead to significant long-term weakness and loss of knee function. Early evaluation is important because delayed treatment can make repair more difficult and recovery more complicated.

During the physical examination, the knee is evaluated for swelling, tenderness, bruising, and range of motion. The physician will assess the patient’s ability to straighten the knee and perform a straight leg raise. Loss of active knee extension strongly suggests a significant tendon injury.

Imaging studies are often used to confirm the diagnosis and determine the severity of the tear. X-rays may show changes in kneecap position or rule out fractures and other bone injuries.

Magnetic resonance imaging is one of the most useful tests for diagnosing tendon tears. MRI provides detailed images of soft tissues and can show whether the tendon is partially or completely torn. It also helps identify the exact location of the injury and evaluate surrounding structures such as cartilage, ligaments, and muscles.

Early diagnosis is important because prompt treatment improves the likelihood of successful recovery. Partial tears may sometimes heal with non-surgical treatment such as bracing and physical therapy, while complete tears often require surgical repair to restore normal knee function. Proper evaluation allows the physician to determine the best treatment plan based on the severity of the injury, the patient’s activity level, and overall health.
