Hip Labral Tears

A hip labral tear is an injury involving the labrum, a ring of cartilage that surrounds the socket of the hip joint. The hip is considered a “ball-and-socket” joint, where the top of the thigh bone fits into the pelvis. The labrum acts like a seal around the socket, helping keep the joint stable while also providing cushioning and smooth movement.

When the labrum becomes torn, the hip may no longer move as smoothly or comfortably. Hip labral tears can affect people of all ages, including athletes, active adults, and even individuals who do not participate in sports.

Not every labral tear causes symptoms. Some people may have small tears without knowing it, while others experience significant discomfort that interferes with walking, exercise, work, or daily activities.

Hip labral tears can develop for several reasons. In many cases, there is not just one single cause. Instead, the injury may result from a combination of anatomy, repetitive movement, and wear within the joint.

One common cause is femoroacetabular impingement, often referred to as FAI. This occurs when the bones of the hip do not fit together perfectly. As the hip moves, abnormal contact between the bones can place repeated stress on the labrum, eventually causing it to fray or tear. FAI is one of the most frequently associated conditions in patients with hip labral tears.

Repetitive motion is another major factor. Athletes who participate in sports involving twisting, pivoting, kicking, or deep hip bending may place increased strain on the labrum over time. This is commonly seen in runners, dancers, soccer players, hockey players, golfers, and weightlifters. Even without a specific injury, years of repetitive movement can gradually damage the cartilage.

Traumatic injuries can also cause a tear. A fall, car accident, sports collision, or sudden twisting injury may place enough force on the hip to injure the labrum. Some people notice symptoms immediately after the event, while others experience a slower onset of pain afterward.

Structural instability within the hip may also contribute. Hip dysplasia, a condition where the hip socket does not fully cover the ball portion of the joint, can increase stress on the labrum because the cartilage must work harder to stabilize the hip.

Degenerative changes associated with aging may play a role as well. Over time, cartilage naturally becomes less resilient. Wear and tear within the joint may gradually weaken the labrum, making tears more likely to occur in middle-aged and older adults.

The symptoms of a hip labral tear can vary significantly from person to person. Some individuals experience only mild discomfort, while others have persistent pain and mechanical symptoms that interfere with daily life.

Pain is the most common complaint. Many patients describe pain deep in the front of the hip or groin area. Others may feel discomfort along the side of the hip, buttock, or upper thigh. The pain is often difficult to pinpoint because the hip joint is located deep beneath layers of muscle and soft tissue.

The discomfort may worsen during certain activities, particularly prolonged sitting, walking, running, pivoting, squatting, or bending at the hip. Some patients notice pain getting in and out of a car or rising from a seated position.

Mechanical symptoms are another important clue. Patients often report clicking, catching, locking, or popping sensations within the hip joint. These symptoms may occur because the torn cartilage interferes with smooth joint movement. Some individuals describe a feeling that the hip is “sticking” or moving abnormally.

Stiffness and reduced range of motion are also common. The hip may feel tight, especially after activity or long periods of sitting. Some patients lose flexibility and find it harder to rotate or fully bend the hip.

In more severe cases, the hip may feel unstable or weak. Certain movements may trigger sharp pain, while other times the discomfort is more of a dull ache that gradually builds throughout the day.

Because the symptoms can resemble other conditions involving the lower back, groin, pelvis, or muscles around the hip, labral tears are sometimes initially mistaken for muscle strains, tendon injuries, or sciatica.

Diagnosing a hip labral tear begins with a detailed medical history and physical examination. An OrthoIllinois orthopedist will ask about the patient’s symptoms, activity level, previous injuries, and what movements tend to trigger pain.

During the examination, the orthopedist evaluates hip motion, strength, flexibility, and stability. Specific movements may reproduce pain or create clicking sensations that suggest irritation within the joint. Imaging studies are usually necessary to confirm the diagnosis and evaluate the overall condition of the hip joint. X-rays are often obtained first. Although the labrum itself cannot be seen on standard X-rays, the images can reveal structural problems such as femoroacetabular impingement, hip dysplasia, arthritis, or other bone abnormalities that may contribute to symptoms.

Magnetic Resonance Arthrography (MRA) provides detailed images of the soft tissues in the hip. During this study, contrast fluid may be injected into the hip joint before imaging, allowing the labrum and surrounding structures to be seen more clearly.

Accurate diagnosis is important because hip pain can have many different causes. Conditions involving muscles, tendons, nerves, arthritis, hernias, and the lower spine can produce symptoms similar to a labral tear. A careful evaluation helps ensure the correct problem is identified so that treatment decisions are based on the true source of pain.
